One-on-One Physical Therapy in Fort Collins
IN-PERSON CARE
In-person appointments provide dedicated time for comprehensive assessment, hands-on treatment, movement, and progressive rehabilitation. Every visit is spent directly with your physical therapist and tailored to what you need that day.
INITIAL EVALUATION
75 minutes · $195
For new patients or those presenting with a new condition. Your evaluation includes a comprehensive assessment, discussion of your goals, and the beginning of an individualized treatment plan.
FOLLOW-UP VISIT
60 minutes · $150
Full-length one-on-one treatment for ongoing care. Depending on your needs, visits may include hands-on treatment, functional dry needling, vestibular or balance rehabilitation, movement retraining, strength training, progressive loading, and exercise progression.
REFRESH VISIT
45 minutes · $110
A shorter visit for established patients who need a focused check-in, treatment progression, or help addressing a specific concern.
New patients should generally begin with an Initial Evaluation. From there, we'll determine the type and frequency of follow-up care that makes the most sense for you.

VIRTUAL CARE
Virtual physical therapy provides access to individualized concussion, vestibular, headache, and movement-based care from anywhere in Colorado.
Many of the systems involved in concussion and dizziness can be effectively assessed and treated virtually, including eye movement and visual function, balance, vestibular function, cervical movement, exercise tolerance, and progressive return to activity.
VIRTUAL INITIAL EVALUATION
60 minutes · $150
For new patients or those presenting with a new condition. A comprehensive virtual assessment to understand your symptoms, identify contributing factors, establish your goals, and build an individualized treatment plan.
VIRTUAL FOLLOW-UP VISIT
30 minutes · $90
Focused follow-up care to assess progress, advance exercises, troubleshoot symptoms, and continue progressing your rehabilitation plan.

Hands-On Treatment
Manual therapy and functional dry needling can be incorporated into your care when appropriate, without separate appointment types or add-on fees.

We believe you should understand the cost of your care before you begin. How payment works depends on your insurance coverage and the type of care you receive.
Medicare & Colorado Medicaid (Health First Colorado)
Forward Physiotherapy participates with traditional Medicare and Health First Colorado (Colorado Medicaid). Coverage, referral, and authorization requirements may vary based on your plan and individual circumstances.

Commercial Insurance
For commercial insurance plans, Forward Physiotherapy primarily operates as a cash-pay practice. Payment is collected at the time of service, and we can provide a detailed superbill that you may submit to your insurance company for possible out-of-network reimbursement.
Reimbursement depends on your individual plan and out-of-network benefits. If you plan to submit for reimbursement, we recommend contacting your insurance company directly to understand your benefits.
For patients using cash-pay care, you can get started without waiting for commercial insurance authorization or a referral.
